As a company, we are always looking to make things as easy for the customer as we can. We are now trialling an online application form for you to apply straight away.
Below, is a guide to filling out the form to assist you without the need to call us, especially if outwith business hours.
First things first, the terms & condtions, the nemisis of every customer, but very important nontheless. Please read these fully, and note down what is required.
After accepting the terms, it’s onto the details. The practitioner is the first name we require, if you don’t have a practioner, we must ask that you contact us by email or phone, to find you one.
If you do have a practitioner, please continue the form as required, with the renter details, and emergency contact. We ask for an emergency contact, just in case we are unable to reach you. If you are a new practitioner, or your paractitioner is new to rentals, please ask them to contact us separately. You may continue to fill out the application in this instance.
Onto the equipment, here you can select the items that you require, taking note of the prices as you go.
Here, you will be asked to upload identiification, we can take any photographic identification and a bill, dated recently.
Below, you can create an estimate of what you may be due as your first payment. It is not a mandatory field.
Lastly, is the usual data protection for you to tick, or leave and then press submit.
Once submitted, the form will make its way to us, and we will create and send a no obilgation quote for you, along with with a secure payment link.
Once paid, we will ask you to confirm the delivery address and provide proof that you have created a standing order for the ongoing monthly payments.
If this guide is followed, the process should take 1 to 3 working days to complete, as long as we have the equipment in stock. We usually carry most devices , but if we need to order one from Germany, this will take up to 14 days to arrive. Your rental will not start until you receive the device in this case.
